Payment & Insurance Information
Co-pays and balances are requested for all office services at the time they are rendered. We accept cash, checks, VISA, MasterCard, American Express, Discover and most insurance plans.
It is extremely important that you bring your insurance cards to your appointment so that we may obtain the necessary and correct information to file all primary and secondary claims for you.
HMOs & PPOs
It is necessary for you to check with your HMO and/or PPO for any special requirements. If you have an HMO and/or PPO that requires a referral from your Primary Care Physician (PCP), we ask that you call your PCP to verify that one has been done. We ask that you bring a copy of the referral to your appointment.
If your services are to be billed to the Bureau of Workers Compensation, the following information must be available to us on or before your first appointment:
- The appropriate active claim number
- Allowed condition and code
- Date of injury
- MCO name, phone number and mailing address
- Name of the employer where the claim occurred
If you have a self-insured claim, we will also need this information.
We are participating physicians with Medicare and accept assignment. We will bill your secondary insurance when needed.
